The Resident Care Coordinator is responsible for coordinating client care related to scheduling of and communication following appointments, transportation, and insurance. In addition, this position guides and assists individuals to live and function as independently as possible in the community and participates in the development and implementation of individual program plans and coordination for the provision of specialized services. This position requires the position to travel between sites on a regular basis.
1.. Coordinate and schedule medical, psychiatric and dental/vision appointments with doctors offices , health care providers, laboratories along with communicating and liaison with physicians , nurses and other relevant behavioral health and health care professionals regarding the medical needs of the resident. ( 65%)
2. Track all physical, health care ,dental , vision appointment . (10%)
3. Prepare and complete all necessary documentation in accordance with program standards within required time frames as required. ( ie. MA 51) (10%)
4. Perform Direct Support Services when needed (10%).
5. Work on computer to perform the above duties - ongoing
6. Part of the after-hours on call system- Ongoing
These are not all encompassed job responsibilities. Other duties may be assigned as necessary due to the needs of the program and residents served.
