Responsibilities: 1.. Coordinate and schedule medical, psychiatric and dental/vision appointments with doctors offices , health care providers, laboratories along with communicating and liaison with physicians , nurses and other relevant behavioral health and health care professionals regarding the medical needs of the resident. ( 65%) 2. Track all physical, health care ,dental , vision appointment . (10%) 3. Prepare and complete all necessary documentation in accordance with program standards within required time frames as required. ( ie. MA 51) (10%) 4. Perform Direct Support Services when needed (10%). 5. Work on computer to perform the above duties - ongoing 6. Part of the after-hours on call system- Ongoing These are not all encompassed job responsibilities. Other duties may be assigned as necessary due to the needs of the program and residents served.
Pre-employment screening: Complete criminal background Name checked in the registries. (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities) Drug testing Education verification and other credentialing based on position requirements. Proof of employment history or references (if required) Positions that require driving Proof of driver's license, driver's insurance, and vehicle, IF required for providing transportation for individuals.
