The Social Worker Assistant will manage resident admission paperwork, care plan development, and resident assessments. They will also oversee the discharge process and facilitate resident care conferences while addressing family concerns.
Candidates must be able to work independently while collaborating with the Social Services Director. The role requires strong organizational skills and the ability to handle sensitive resident and family interactions.

Part-time, flexible day schedule between the hours of 8:00am - 5:00pm (hours per shift is also flexible). $400 sign-on bonus - $27.93 - $34.23. This is a new position that FHS plans to fill in May/June 2026.
