The Medical Assistant Coordinator proactively scrubs charts to identify care gaps and coordinates with clinical teams to ensure preventative screenings and referrals are completed. They also conduct patient outreach to address barriers to care and maintain accurate documentation within the electronic health record.
Candidates must have at least 2 years of experience as a Medical Assistant in a primary care setting and a strong understanding of healthcare quality measures. Proficiency in electronic health records, Microsoft Office, and excellent organizational skills are required for this role.
Description Redwood Coast Medical Services (RCMS) is seeking a Medical Assistant Coordinator (MAC) to join our Population Health and Quality team.
· Support Partnership and PHMI initiatives focused on accountable care and quality performance. · Be assigned to 1–3 provider/MA teams and proactively scrub charts 1–4 weeks before patient visits. · Identify care gaps and ensure screenings, labs, and referrals are completed prior to encounters. · Conduct patient outreach to support completion of preventative screenings and address barriers such as transportation or access challenges. · Document all outreach and updates accurately in the electronic health record. · Ensure loop closure on referrals and embed results correctly in patient charts. · Partner with Patient Services to schedule timely follow-ups. · Participate in quality meetings and performance reviews to support continuous improvement. · Serve as a workflow and documentation resource to clinical teams.

· 2+ years of experience as a Medical Assistant in a primary care setting (FQHC or rural health preferred). · Strong understanding of preventative care workflows and healthcare quality measures (HEDIS, Medi-Cal measures, Population Health). · Experience working within an electronic health record (NextGen preferred). · Strong documentation, organizational, and follow-through skills. · Advanced Microsoft Office skills (Word and Excel required). · Ability to manage confidential information with discretion. · Excellent communication and collaboration skills. · Strong time management and ability to work independently while supporting a team.
