About the Role

The Case Manager (“CM”) is an integral member of Community Reach Center’s ACT Team (“Division”) Division. The CM provides clinical and support services to consumer and/or their families through integrated behavioral healthcare services. The CM will utilize a Psychosocial Rehabilitation Model that emphasizes assisting persons to explore and develop social, occupational, leisure, and living skills. Provides services that supports consumers in living in the community successfully including linking consumers with resources to obtain and/or learn basic needs. Additionally, the CM will have other duties and responsibilities as determined from time to time by the Program Manager. Essential Functions: Provides prevention/early intervention, case management, family psychoeducation, and other clinical interventions to consumers and families as appropriate to the position. Support and closely coordinate mental health care with the consumer’s clinical care team and the primary care provider, supports and family members as needed. Provide brief interventions using evidence-based techniques such as Motivational Interviewing, or other treatments appropriate to support consumer in engagement in recommended services, in maintenance of housing, in making and keeping medical appointments and support healthy living activities. Track patient follow-up and clinical outcomes using Electronic Health Record system (HER). Document in-person and telephone encounters in EHR and use the system to identify and re-engage patients. Attend collaborative treatment team meetings and reviews of consumers Provides therapeutic crisis intervention and emergency services as needed Assist eligible consumers in gaining access to needed medical, social, educational, and other services. Provide an assessment of needs, service plan development, referrals/linkage, and monitoring/follow ups for consumers. Facilitates and contributes to the accomplishment of tasks by consumers, empowers consumers to reach optimum level of achievement, and sets a positive example and provides role modeling as well as technical expertise to accomplish goals. Attends and participates in group sessions, in-service education and training opportunities, meetings, and special events in order to utilize opportunities of professional growth. Provide transportation for consumers in agency vehicles when clinically indicated. Participates in, and utilizes opportunities that provide professional growth through attendance at agency in-services and other conferences Will work closely with outside organizations on resources and care coordination. Assists in pinpointing program and management problems and participates in problem resolution, implementation, and evaluation. Uses identified agency key performance indicators to track success, engagement, and other factors leading to successful completion of treatment.

Requirements

The position requires a bachelor's degree in psychology, social work, or a related field and a valid Colorado driver's license with a clean driving record. Six months of experience working with adults with serious and persistent mental illness is preferred, along with strong communication and organizational skills.

Qualifications

Requires bachelor’s degree in psychology, Social Work, or directly related field 6 months of experience working with adults who have mental illness, particularly those individuals diagnosed with a serious and persistent mental illness preferred, but not required. Communication, organization, time management and clinical skills. Ability to work independently in a community-based setting. Valid Colorado Driver’s license with a clean driving record.
