The Service Coordinator links elderly and disabled residents to essential community supportive services to promote independent living. They provide case management, facilitate resource referrals, and educate residents on service availability and rights.
A bachelor's degree in Social Work, Gerontology, Psychology, or Counseling is preferred, though relevant work experience may be accepted. Candidates must have at least two years of experience in management functions and a strong knowledge of senior support resources.
The Social Services Coordinator is responsible for linking the elderly, especially those who are frail and disabled, or the disabled residents of the project to the supportive services they need to continue living independently.
Service coordination means the activity of linking a project resident to needed supportive services or medical agencies in the general community. Additionally, the term may cover case management, both formal and informal, in which the service coordinator assesses service needs, determines eligibility for public services, and makes resource allocation decisions.
Starting pay ranges from $22.90 - $30.30 per hour. Factors such as scope, responsibilities, experience, education, job-related skills, internal equity, and market considerations may influence base pay.
• A Bachelor of Social Work or degree in Gerontology, Psychology, or Counseling is preferable; a college degree is fully acceptable. However, individuals without a degree, but with appropriate work experience, may be hired.
• Two years of experience with progressive responsibility in management functions.

Exerting up to 20 pounds of force frequently, and/or a negligible amount of force constantly to move objects. If the use of arm and/or leg control requires exertion of forces greater than that of sedentary work and if the worker sits most of the time, the job is considered light work.
The final candidate must successfully pass Eskaton's post offer, pre-employment testing which includes a criminal background check, drug test, TB screen test and health screen. All new hires are required to sign an arbitration agreement as a condition of employment.
