About the role

The Patient Care Associate ensures the safety and well-being of clients by providing physical support with daily activities and monitoring vital signs. They also document patient health data, maintain medical supplies, and assist the nursing team with various medical procedures.

Job purpose

The Patient Care Associate is responsible for ensuring the safety and well-being of persons served/clients as they go through the rehabilitation process.

Essential duties and responsibilities

Answers patient calls and determines how to best assist them. Provides physical support for patients or residents with daily activities such as: turning or moving patients for treatment of care, bathing patients, feeding patients, taking care of wounds and grooming patients by brushing their hair, teeth, shaving them, etc. Documents patient’s food and liquid intake. Checks vital signs such as blood pressure and heart rate for patients. Records vital signs, including blood pressure, pulse, temperature, and respiration rate as requested by staff. Observes patient behavior, including complaints, or physical symptoms to nurses. Ensures Universal Precautions and Safety Procedures of the center. Maintains confidentiality at all times and displays a professional disposition towards fellow staff. Brings policy/personnel issues to supervision as they arise. Gathers and stocks medical supplies for needed purposes. Cleans patient rooms and changes bed linens. Assists with some medical procedures such as UDS, Lab draws etc. Transports patients as needed. Other duties may be assigned.

Required skills or abilities

Ability to interact with a multi-disciplinary team to assist in facets of the recovery program relating to physical health. Have the ability to communicate with the Case Manager and assist in ensuring the safety of the client. Flexibility to adapt to schedule changes and assumption of responsibilities not delineated in the job description which is related to work as a member of an addiction treatment team. Attention to detail and organization a must. Computer literate with experience in a Windows environment. Ability to work independently with minimal supervision Ability to prioritize work and work-related responsibilities.

Qualifications/Education/Experience

Must have High School Diploma or GED. Certified Nursing Assistant preferred but not required. Substance abuse experience preferred but not required. Current CPR/BLS/First Aid.

Physical demands

Physical demands described are representative of those that must be routinely met by and employee to successfully perform the essential duties of this position. Reasonable accommodations may be made to enable qualified individuals with disabilities to perform the essentials duties. Ability to lift minimum 25 pounds. Ability to perform job responsibilities in a facility with multiple levels and without elevator or mechanical transportation. Operates a computer and keyboard.

Requirements

Candidates must have a High School Diploma or GED and current CPR/BLS/First Aid certification. Experience in substance abuse treatment and a Certified Nursing Assistant (CNA) credential are preferred but not required.
