What You'll Do Deliver skilled nursing visits including wound care, catheter care, and medication administration Monitor patient vitals and health status, flagging changes to the supervising RN or physician Educate patients and families on medications, diagnoses, and how to thrive between visits Implement individualized care plans under RN supervision and collaborate with the full care team Coordinate with RNs, therapists, and home health aides to keep care seamless and patient-focused Keep documentation accurate and timely — because great care deserves a great paper trail What You Bring Active LPN license in the State of Florida (Compact license accepted) 1+ year of home health or clinical nursing experience preferred Current CPR/BLS certification Valid Florida driver's license and reliable transportation Proof of TB test, required immunizations, and health clearance Strong communication skills and a patient-first mindset
