The LPN provides direct nursing care, health monitoring, and medical oversight for individuals with developmental disabilities in residential settings. They are responsible for medication management, coordinating medical appointments, and collaborating with interdisciplinary teams to implement health plans.
Candidates must hold a current, active Licensed Practical Nurse (LPN) license and possess valid CPR/First Aid certification. Experience in developmental disabilities, behavioral health, or home health settings is required, along with the ability to pass background checks and maintain a valid driver's license.
Current, active Licensed Practical Nurse (LPN) license in good standing. Valid CPR/First Aid certification (or willingness to obtain). Ability to pass criminal and abuse registry background checks. Valid driver’s license, acceptable MVR and current auto insurance. Strong communication skills and ability to work with individuals with diverse physical and cognitive abilities. Experience in developmental disabilities, behavioral health, group home, home health, or similar settings. Knowledge of person-centered planning and trauma‑informed care. Experience with electronic health record (EHR) systems. Knowledge of Therap a plus. Work Environment Community‑based group home setting, day center and independent living environments. Requires lifting, transferring, or supporting individuals as needed. May include rotating shifts, on‑call duties, or weekend/holiday coverage depending on agency needs.
