The Case Manager provides advocacy, resource linkage, and transportation services to participants while developing personalized treatment and crisis stabilization plans. They are responsible for maintaining relationships with partnering agencies and documenting all interactions using DAP note formatting.
Candidates must hold an associate's degree in human services or a related field and possess a valid Alaska driver's license. If in recovery, applicants must have one year of continuous sobriety and must be able to pass a state and federal background check.

Responsible for providing case management services to participants within the program and will act as an advocate for the participant providing resources, case management, transportation as well as working with participants on pre- and post-treatment planning. ROLES AND RESPONSIBILITIES: Maintains a relationship with all partnering agencies and referral sources. Efficiently document all billable interactions into OPUS, utilizing DAP note formatting. Coordinates with referral sources, involved family members, community agencies and other treatment providers to provide the best treatment and case management options for the individual participant’s needs. Provides motivational interviewing, social, and emotional support to participants entering and engaging in program. Create crisis stabilization plans as needed for participants exploring treatment and coordinate assistance from peer supports to accomplish immediate/crisis goals. Assists participants in identifying case management services; as needed, in accordance to best practices to provide linkage and advocacy that participants need. To include coordinating referrals that pertain to employment, mental health, housing, medical with local community clinics, social service agencies, continuing care and provide follow-up on all referrals made. Works with participants to develop and update personalized plans prior to program completion to ensure participants leave the program with a clear plan for continued housing, employment, support, and treatment needs. Assist participants in exploring available grants to aid them in getting funding for housing, transportation, education, and other needed items. Attends all relevant trainings and direct support meetings. Participate in training in areas related to improvement of service delivery. Attends staff meetings and works collaboratively with other staff in case staffing/review. Transport participants on assigned activities, case management services and outings as needed. Maintains a respectful relationship with participants, former participants, family members and peers. Performs other duties as needed and/or assigned.
AND EDUCATION REQUIREMENTS: Associate's degree in human services or related field. An equivalent combination of education and experience may be accepted. If in recovery, one year of continuous sobriety prior to hire and active and continuing participation in a recovery support program, such as a 12-step program. Must obtain CDC II within 3 years of hire. Valid Alaska driver’s license and insurable under TRUE NORTH RECOVERY INC’s automotive insurance, which requires a driver to be at least 21 years of age. Continued employment is contingent upon receipt of a satisfactory state and federal background check.

Ability to work independently with minimal supervision Ability to work under pressure and prioritize workload. Strong public speaking and organizational skills. Strong problem-solving skills. Observes participants rights to confidentiality. Ability to coordinate multiple activities. Equivalent combination of HSD or GED and experience may be accepted.
