The Licensed Clinical Social Worker conducts comprehensive mental health assessments and develops individualized treatment plans for clients. They provide individual, group, and family therapy while collaborating with interdisciplinary teams to ensure holistic patient care.
Candidates must hold a current state licensure as a Licensed Clinical Social Worker (LCSW). A minimum of two years of clinical experience providing mental health services is required.
Hometown Health Center (HHC) has a proud history of delivering high-quality healthcare in Central Maine. Originally founded in 2003 as Sebasticook Family Doctors (SFD), the organization began after Sebasticook Valley Hospital received a Federally Qualified Health Center (FQHC) grant. Before becoming an FQHC, the organization operated as The Family Doctors, located in Dexter and Canaan.
In 2016, SFD rebranded as Hometown Health Center to better reflect our commitment to community-focused care. Today, we serve patients at three locations: Dexter, Palmyra and a School-Based Health Center at Nokomis Regional High School for RSU19 students.
Our services have grown to meet the needs of our communities. We proudly offer medical, behavioral health and dental care, including outreach to local schools to provide dental services to children. Everyone at HHC shares one goal: building healthier communities. We work toward this every day by offering high-quality medical care, preventive services, care coordination, same-day appointments, and acute care.
HHC provides more than 32,000 medical, dental and behavioral health visits annually to over 8,200 patients. We use an integrated electronic medical record system and patient portal to streamline care and ensure continuity across all services.

About the Role: Licensed Clinical Social Worker (LCSW)
The Licensed Clinical Social Worker (LCSW) plays a crucial role in providing mental health services and support to individuals and families facing a variety of challenges. This position involves conducting assessments, developing treatment plans and delivering therapeutic interventions to promote emotional well-being and resilience.
The LCSW collaborates with other healthcare professionals to ensure a holistic approach to patient care, addressing both psychological and social factors. Additionally, this role requires maintaining accurate documentation and adhering to ethical standards and professional regulations. Ultimately, the LCSW helps empower clients to achieve their personal goals and improve their overall quality of life through effective counseling and support.
