The Case Manager coordinates care and connects individuals with mental health and substance use challenges to vital community resources. They provide mobile in-home services, facilitate person-centered plans, and assist with benefit acquisition to support recovery journeys.
Candidates must hold a Bachelor's degree and have at least two years of paid post-degree experience in mental health case management. A valid driver's license, reliable vehicle, and specific certifications like SOAR and CPR are required.

You’ll work with veterans, individuals facing mental health and substance use challenges, and underserved communities—helping them access benefits, housing, medical care, and more.
Department: CCBHC- Hudson County
Salary: $48,000- $50,000
Location: Hoboken, NJ
Work hours: Monday-Friday (3 days 9:00am-5:00pm, 2 days 12:00pm-8:00pm)
Provide wrap around service, linkage, service coordination, and outreach as needed. SOAR trained to support rapid benefit acquisition and approved to do presumptive eligibility to ensure benefits for acquiring medication and services. Teaching and modeling of recovery concepts and skills to support each person’s individualized recovery journey, community integration, and improved health outcomes.
