The Clinical Pharmacist provides optimal therapeutic medication regimens for various patient populations including acute, critical, and oncology care. They are responsible for reviewing and intervening in medication therapy, dispensing medications, and providing drug information to healthcare professionals.
Candidates must hold a current California Registered Pharmacist License and a Doctor of Pharmacy degree. BLS and ACLS certifications are required, with residency completion and board certification preferred.
POSITION SUMMARY Under the direction of the Pharmacy Director, the Clinical Pharmacist provides optimal therapeutic medication regimens for Acute Care, Critical Care, Oncology, long term care, and Transitions of Care for adult, pediatric and geriatric patients. The Clinical Pharmacist reviews, assesses and intervenes in patient medication therapy, processing medication orders, dispensing medications, providing drug information to health care professionals and education to patients. Oversees the application of pharmacy law with hospital/pharmacy policies and procedures on a day-to-day basis. POSITION QUALIFICATIONS Education/Licensure/Certification: Current State of California Registered Pharmacist License required. Doctor of Pharmacy Degree or equivalent combination of education and experience required. Current Basic Life Support (BLS) certification required. Current Advanced Cardiac Life Support (ACLS) certification required. Completion of ASHP Accredited Residency or equivalent preferred. Board Certification preferred.
Minimum of 6 months experience in acute care hospital or new residency graduate preferred.
