Responsibilities

Heritage Home Health Physical Therapists (PTs) work with patients, their families, and other healthcare professionals in the patient’s place of residence. In addition to performing visits and completing coordination of care, the PT is the point of contact for all disciplines involved with providing care to the patients. The PT follows agency, state, and federal policies and procedures. The PT will consult with the patient’s primary care physician and attend meetings as required.

PTs must meet the following requirements

Be currently licensed as a PT in the state of Idaho A minimum of one year of clinical experience is preferred but not required Demonstrate knowledge and skill in PT practice CPR (BLS) professional certification General knowledge of computers and the ability to learn industry-specific software and documentation applications Possess a valid state driver’s license and automobile liability insurance
