The Occupational Therapist administers skilled therapy treatments focused on activities of daily living, functional mobility, safety, cognition, and upper extremity function based on physician's orders. This role involves evaluating residents, developing personalized treatment plans, maintaining accurate documentation, and collaborating with the interdisciplinary team.
Candidates must possess an active New York State Occupational Therapist license and a graduation from an approved occupational therapy school, with two years of experience preferred. Essential requirements include strong communication skills, commitment to evidence-based care, and the ability to work well with geriatric and chronically ill residents.
Occupational Therapist: Initiates skilled occupational therapy treatments in accordance with physician’s orders; maintains required contact with physician. Maintains written evaluations, progress notes, MDS records and discharge summaries for individuals in occupational therapy program. Confers with other health care staff regarding special needs of indviduals to enhance rehabilitation or maintenance of function. Provides input for Care Planning meetings and attends during absences of the Director of Occupational Therapy. Maintains statistical records of indviduals’ treatments. Assists with in-service training programs upon request. Records and updates individual’s care plan. Submits requisition for supplies needed in the treatment of individuals to Director of Occupational Therapy. Assumes all duties of the Director of Occupational Therapy, if applicable, during absences including supervision of COTA’s and Occupational Therapy Aides. Occupational Therapist: Graduation from an approved school of occupational therapy required. State license to practice occupational therapy required. Two years’ experience in occupational therapy preferred. Interest in occupational therapy treatment programs for the geriatric and subacute populations utilizing a team approach. Ability to direct others necessary. Ability to communicate well verbally and in writing required. Current, valid CPR/BLS credential Sense of responsibility and ability to work well with residents, families, and other staff members required. Qualities of maturity, diplomacy, and ability to work well with others required. Ability to work with geriatric and chronically ill residents, as well as other professionals in a friendly and helpful manner necessary. Ability to perform tasks to established standards of excellence required. This position requires regular interaction with residents, coworkers, visitors, and/or supervisors.
