The Nurse Practitioner will conduct in-home visits, perform physical assessments, and develop individualized care plans in collaboration with the medical team. They are also responsible for prescribing medications, managing symptom control, and providing education to patients and families regarding end-of-life care.
Candidates must hold a Master's degree from an accredited Nurse Practitioner program and possess current state licensure and CPR certification. At least one year of experience as an NP and one year in hospice or palliative care is required, along with a valid driver's license.
