The Targeted Case Manager will conduct community-based needs assessments and provide recovery-focused support to individuals experiencing homelessness or transitioning from institutional settings. They will facilitate care coordination, advocate for client needs, and maintain high-quality documentation while participating in interdisciplinary treatment team meetings.
Candidates must hold a Bachelor's degree in Social Work, Psychology, Human Services, or a related field and possess a valid Georgia Driver's License. Proficiency with electronic medical records and strong care coordination skills are required, with preference given to those with prior experience in behavioral health or residential services.
