The Case Manager provides community-based behavioral health services, including assessing client needs and monitoring recovery plans. They act as a single point of contact to help clients obtain essential resources like housing, healthcare, and employment.
Candidates must hold a bachelor's degree in a relevant social science field or possess equivalent experience as defined by the organization. The role requires the ability to manage a caseload of up to 30 clients and participate in a 24/7 on-call rotation.

Pennsylvania Child Abuse, Criminal and fingerprint-based federal criminal history; Verification that employee is not on any Medicaid/Medicare Exclusion list
At the discretion of the Personnel Officer, additional related experience and/or education may be substituted instead of the requirements specified under Education and Experience
