The Nurse Practitioner will provide care for an ambulatory population with chronic medical conditions as part of a multi-disciplinary team. They are responsible for maintaining clear communication, building collaborative relationships, and actively contributing to the patient experience.
Candidates must have completed a Family Nurse Practitioner program and hold a valid Nurse Practitioner Certification. A minimum of one year of related experience is required for this position.
Provide inpatient only palliative care consultations for hospitalized patients with serious or life-limiting illnesses Lead and participate in family meetings and goals-of-care discussions Collaborate with interdisciplinary team members including nursing, social work, chaplaincy, and case management Support transitions of care, including coordination with hospice when appropriate Participate in quality improvement initiatives and program development
