Description Are you passionate about helping older adults maintain their independence and improve their quality of life? Do you thrive in a collaborative, interdisciplinary care environment? PACE North is looking for a dedicated and compassionate Physical Therapist to lead and deliver high-quality care to the aging population - both at our Center and in participants' homes. We are seeking a licensed Physical Therapist to join our team on a temporary, fixed-term basis to provide coverage during a maternity leave.

Position Details

Temporary / Fixed-Term Approximate duration: 4 months Schedule: Full-Time, hours negotiable, Monday - Friday What You’ll Do: As a Physical Therapist at PACE North, you’ll be responsible for helping participants maximize their mobility and independence. You’ll: Conduct initial and periodic assessments of participants’ physical and functional status Develop personalized care and treatment plans, including therapeutic exercise, gait training, and mobility support Collaborate with an interdisciplinary care team and communicate changes in participant condition Recommend and evaluate the need for durable medical equipment Train family members, caregivers, and staff on implementing therapy strategies Contribute to participant discharge planning and continuity of care across settings Why Join PACE North? Requirements

What We’re Looking For: Education Bachelor’s in physical therapy, Master’s, or Doctorate Licensure/Certifications: Licensed Physical Therapist in Michigan CPR Certification (We can train) Valid Michigan Driver’s License Skills & Qualities: Strong clinical competency in geriatric care Basic computer proficiency Ability to work independently and collaboratively Clear communication and documentation skills Commitment to participant-centered care and quality improvement
