The Support Coordinator manages case activities for individuals with intellectual or developmental disabilities to ensure they receive necessary services and resources. They coordinate supports among various providers to help individuals function optimally within their community.
A high school diploma is required, with a bachelor's degree in a human services field preferred. Candidates must have at least one year of experience working with individuals with disabilities and maintain active QDDP registration.
The Support Coordinator provides targeted case management/support coordination activities for individuals diagnosed with Intellectual Disabilities or Developmental Disabilities (ID/DD). Works as part of the team to ensure individuals are linked with necessary services, supports, and resources. Ensures supports are coordinated amongst providers as needed to help individuals live and function optimally in the community.
***$3,000 Sign on Bonus***
Starting Salary: $39,000-Depending on Experience
Additional compensation will be considered for any new employees who speak multiple languages.
