The Social Worker performs psychosocial assessments of patients and families to develop and implement effective care interventions. They also provide psychoeducation and counseling while serving as a key member of the interdisciplinary group.
The role requires a professional capable of providing expert psychosocial support within a hospice setting. Candidates must demonstrate strong collaboration skills and a commitment to professional growth and patient-centered care.
