Key Responsibilities
Assist individuals experiencing homelessness to access safe and stable housing.
Conduct comprehensive assessments, develop person-centered service plans, and monitor progress toward housing and self-sufficiency goals.
Provide case management, advocacy, and supportive counseling tailored to residents’ needs.
Facilitate linkages to community resources, benefits, childcare/preschool, income supports, and other essential services.
Offer crisis intervention to ensure the safety and security of residents, staff, and visitors.
Support residents in budgeting, financial literacy, and household management.
Participate in PREP classes, resident meetings, and discharge planning to ensure smooth transitions to permanent housing.
Maintain accurate and timely documentation in accordance with agency standards.
Collaborate with colleagues, community partners, and other service providers to ensure holistic care.
Utilize trauma-informed, Housing First, and harm reduction approaches.
