The Case Manager will assist homeless individuals in accessing stable housing by conducting assessments, developing service plans, and providing tailored case management and supportive counseling. Responsibilities also include facilitating linkages to community resources, offering crisis intervention, and supporting residents with financial management.
Candidates must possess a Bachelor's degree in Social Work, Human Services, or a related field, or an equivalent combination of education and experience, along with a valid driver's license. Preferred qualifications include experience in case management, shelter services, or working with the homeless population, alongside proficiency in relevant software.
