The Navigator will conduct outreach, educate, and facilitate the enrollment of uninsured patients into PCAN 'medical homes' under the direction of the Program Manager and Director of Case Management. This involves performing administrative and casework duties, including interviewing applicants to determine eligibility for program assistance and linking residents with appropriate service agencies.
Candidates must possess an Associate's Degree from an accredited institution and two years of experience in social, health, or public services, or an equivalent combination of education and training. Required abilities include computer competency, knowledge of word processing/spreadsheet applications, and the capacity to work well with people.

1. Associates Degree from an accredited institution of higher learning 2. Two (2) years of experience in social, health, or public services, or an equivalent combination of relevant education, training, and experience
