Coordinates care management services for family members and caregivers, focusing on high-risk patients and social barriers to care. Acts as a liaison between hospitals, providers, and community resources to ensure seamless patient-centered coordination.
Requires a high school diploma and 1-3 years of relevant experience, with a bachelor's or master's degree strongly preferred. Experience in health homes or care management and a NYS Driver's License are highly desired.
Coordinates and participates in activities related to Care Management services to family members and caregivers.
