About the role

Provide comprehensive nursing services and health checks to first-time expectant parents and their children up to age two. Manage a caseload of 25-30 clients while implementing the Nurse-Family Partnership model to support vulnerable families.

Description SUMMARY The Nurse-Family Partnership Nurse Home Visitor is responsible for providing comprehensive nursing services to women and their families eligible for the NFP Program. Nurse Home Visitors are pivotal in transforming the lives of vulnerable babies, mothers, and families by implementing and maintaining fidelity to the model.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.

Shares resources, performs health checks, and partners with first-time expectant parents during pregnancy, and until the child is two Community-based position visits parents in their home or community location.

Provide support to parents to uncover their innate strengths and skills as parents.

The NFP Nurse will carry a caseload of 25-30 clients.

Extensive model training by the NFP National Service office and ongoing reflective supervision with site supervisors.

Individuals must be compassionate nurses with experience in maternal health, labor and delivery and community health nursing.

Travel required visiting families, attending training and meetings, and adhering to company and program guidelines.

Position covers travel within New Castle County Ability to work flexible hours, including nights and weekends, as needed.

EDUCATION, LICENSURE and EXPERIENCE Must be a registered nurse with a minimum of an Associate's degree in Nursing, with a current RN license (in good standing).

Experience in public health, community, and/or maternal-child health, required.

Experience with labor and delivery, desirable.

Home visiting experience preferred.

Bi-lingual (English/Spanish), desirable.

Current CPR required.

Must have a valid driver’s license, which meets the requirements of the agency’s insurance carrier.

Qualifications

Strong interpersonal, verbal/written communication skills required Ability to work cooperatively and respectfully with others, including people of diverse cultural backgrounds and ethnic groups. Demonstrate ability to establish and maintain effective working relationships with clients, co-workers, and community. Ability to help others learn through a variety of methods· Knowledge of community resources.· Ability to work independently as well as a member of the team. Experience with computers, including Microsoft Office.· Must have valid driver’s license, which meets the requirements of the agency’s insurance carrier.

Requirements

Must be a Registered Nurse with at least an Associate's degree and a valid RN license. Experience in public, community, or maternal-child health is required, along with a valid driver's license and CPR certification.
