Review patient charts and medical history Develop a personalized treatment plan Teach therapeutic exercises and stretches to improve patient’s conditions Use hands-on therapy to promote healing Maintain patient records by keeping track of goals and progress while adapting treatment plans accordingly Work closely with a supportive patient care team including physicians, nursing, social services, pastoral care, home health aides and others for your assigned caseload. New York State PT license Minimum of one year of recent hands on experience as a Physical Therapist. Valid New York State Driver’s License and reliable transportation

Candidates must possess a valid New York State Physical Therapist license and a valid New York State Driver's License with reliable transportation. A minimum of one year of recent hands-on experience as a Physical Therapist is required.
