You’ll be the link between patients, caregivers, physicians, and the clinical team—delivering skilled nursing, developing care plans, and offering emotional support while helping families navigate this important time.

  • What You’ll Do
  • Deliver skilled, compassionate care to patients in their homes
  • Perform assessments and develop individualized care plans
  • Educate and support patients and families/caregivers
  • Collaborate with physicians, LPNs, CNAs, and community providers
  • Participate in on-call rotations for urgent or standby visits

This is a role for nurses who want autonomy, impact, and the chance to make a real difference in patients’ lives while enjoying a supportive team environment.

  • What You Bring
  • Current Registered Nurse (RN) license in Washington
  • Home health or hospice experience preferred
  • Current BLS certification (AHA)
  • Licensed driver with insured transportation
  • Compassion, communication skills, and clinical expertise

Requirements
