Collaborate with a multidisciplinary team to deliver coordinated care and conduct home visits for patients. Responsibilities include assessing care plans, administering medications, and maintaining accurate clinical progress notes.
Candidates must hold an active LPN state license and a valid driver's license. A minimum of two years of applicable nursing experience is required, and bilingual skills in Spanish are preferred.
Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs Manage emergency situations in a prompt, precise, and professional manner Complete medication administration and treatment needs for all assigned patients Provide nursing care in accordance with applicable regulations and Company policy Prepare and maintain accurate and complete clinical and progress notes on a timely basis
