• We are seeking a Licensed Speech-Language Pathologist (SLP) to join our Therapy Department at Jamestowne, a short-term rehab facility with an additional focus on outpatient services, with Community First Solutions.
• In this role, you’ll provide comprehensive speech-language pathology services including evaluations, treatment planning, ongoing reassessments, and discharge planning while collaborating with residents, families, and healthcare professionals.

• You bring at least two years of SLP experience, preferably in geriatrics or long-term care.

Responsibilities

• Administers physical therapy in compliance with Federal, State, local, and Community First Solution’s standards, policies, and guidelines ensuring quality care and service to Community First Solution’s residents.
• Interacts with health professionals, residents, families, and support groups to ensure the maximum rehabilitation, while serving as a client advocate at all times.
• Evaluates residents by administering muscle, neurological, gait, and functional tests.
• Establishes treatment goals based on the evaluation of the resident in conjunction with the resident’s needs.
• Develops treatment programs and provides ongoing assessments and modification as changes in resident’s condition occur.
• Prepares, transmits, and maintains timely and accurate records to include the initial evaluation, progress notes, discharge summary, and attendance records.
• Complies with OSHA standards and ensures the safety of the resident in their care at all times.
• Adheres to infection control policies, professional therapeutic services policies and all Community First Solution’s policies and procedures.
• Adhere to all policies and procedures of the organization and all federal and state laws regarding the privacy of protected health information.
• Adhere to all policies and procedures of the organization regarding the use of technology and technology-based information.
• Responsible for welcoming and on-boarding new employees.

Qualifications

Education

• Bachelor’s degree from an accredited college in Physical Therapy (Master’s degree preferred).
• Licenses or other required certifications: Current Ohio Physical Therapy License.

Experience
