• We are seeking a Licensed Physical Therapist (PT) to join our Therapy Department at Jamestowne, a short-term rehab facility with an additional focus on outpatient services, with Community First Solutions.
• In this role, you’ll provide comprehensive physical therapy services including evaluations, treatment planning, ongoing reassessments, and discharge planning while collaborating with residents, families, and healthcare professionals.
• You’ll develop individualized care programs, modify treatments as resident needs change, and ensure optimal functional outcomes.
