The Nurse Practitioner provides primary care, acute, and chronic disease management for participants under the supervision of a Physician-In-Charge. Responsibilities include performing comprehensive assessments, prescribing medications, and collaborating with an interdisciplinary team to coordinate care.
Candidates should have 3-5 years of clinical RN experience with a preference for geriatric expertise and at least one year of experience with frail or elderly populations. A current Michigan NP license in good standing and compliance with BLS and infection control requirements are mandatory.
