Develops and implements social service programs for skilled nursing residents while ensuring compliance with state and federal guidelines. Acts as a resident advocate and manages psychosocial needs, discharge planning, and insurance counseling.
Requires a Bachelor's degree in social work or human services. Experience in a skilled nursing environment is preferred along with strong communication and collaborative skills.

Part Time- 20 hours per week. Candidates will have flexibility in their work schedule to the extent that they can successfully perform the required duties of the position.
Experience based rates $26.00-$30.00/hour.
