About the role

Conduct evaluations and establish plans of care to improve speech and language function for home health patients. Coordinate with physicians and instruct families on treatment regimes while maintaining clinical documentation.

We are a locally owned and operated home health agency is looking to add staff to its current long-term and experienced team. Operating in the Valley since 2013, Valley Regional Care has been a leading provider with 500+ patient census. Hourly pay structure that allows therapists to make more income than on a per visit basis.

Service Area

Fresno, Clovis, Madera, Sanger, Selma, Kerman, Reedley, Fowler, Del Rey, Tulare, Dinuba, Lemoore, Riverdale, Hanford, Visalia, and more! Our main office is located in Fresno but we fully support employees that do not live in Fresno.

Essential Job Functions/Responsibilities Conducts appropriate evaluations, assesses home environment (as appropriate), identifies equipment needs relative to speech/language function.

Identifies functional speech defects and establishes a plan of care to improve patient’s function.

Administers speech therapy program utilizing specialized therapeutic technique and/or equipment.

Recommends communication devices/aids as indicated.

Confers with referring physician and other organization personnel to ensure coordinated and comprehensive care contributes to the total plan of care.

As appropriate, participates in case conferences, inservices and other meetings required to ensure coordinated and comprehensive care.

Prepares and submits notes per organization policy.

Maintains clinical competency in speech language pathology practice and theory.

Identifies patient and family/caregiver needs for other services and refers as appropriate.

Instructs patient, family/caregiver and other Organization health care personnel in the patient’s treatment regime as indicated.

May supervise home health aide in procedures directly related to the speech–language therapy plan of care.

Performs other duties as required to facilitate the delivery of speech language pathology services.

On occasion, one may be required to bend, stoop, reach and move patient weight up to 250 pounds lift and/or carry up to 30 pounds The above statements are only meant to be a representative summary of the major duties and responsibilities performed by incumbents of this job.

Position Qualifications Possesses a master’s or doctoral degree in speech-language pathology (SLP), and who meets either of the following requirements: Is licensed as a speech language pathologist by the State in which the individual furnishes such service; or Must successfully completed 350 clock hours of supervised clinical practicum (or is in the process of accumulating such supervised clinical experience).

Performed not less than 9 months of supervised full-time SLP services after obtaining a master’s or doctoral degree in SLP or a related field; and Successfully completed an approved national examination in SLP A minimum of one year's clinical experience in Speech–Language Pathology.

Demonstrates good verbal and written communication, and organization skills.

Possesses and maintains current CPR certification.

Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Requirements

Requires a master's or doctoral degree in speech-language pathology and a valid state license or equivalent clinical practicum. Candidates must have at least one year of clinical experience, current CPR certification, and a reliable insured vehicle.
