Responsibilities

• Conduct regular health assessments and monitor vital signs of patients in home settings.
• Administer medications and treatments as prescribed by physicians.
• Provide education and support to patients and their families regarding care plans.
• Document patient progress and report any changes to the healthcare team.
• Collaborate with interdisciplinary teams to optimize patient care.
• Ensure compliance with all safety and health regulations.
• Assist with personal care tasks, including bathing, dressing, and mobility support.
• Maintain accurate and timely records of patient care activities.

Requirements

• Current LPN license in the state of Maryland.
• Minimum of 1 year of nursing experience, preferably in home care or similar settings.
• Strong assessment and clinical skills with attention to detail.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Compassionate demeanor and a passion for patient-centered care.
• Basic Life Support (BLS) certification is preferred.
• Reliable transportation and willingness to travel to patient homes.
