The Community Health Worker provides education, training, and support to children and families within behavioral and mental health programs. They operate across various community settings to link individuals with necessary resources and evidence-based crisis support.
Requires a high school diploma and Michigan CHW certification, along with at least one year of experience working with children and families. Candidates must possess a valid driver's license and be able to meet specific physical demands including lifting up to 35lbs.

Under the direction of the Supervisor of Family Support Services, or designee, the Community Health Worker works as an integral member of behavioral/mental health programs within Genesee Health System.
Work occurs in a variety of venues and locations, including but not limited to schools, healthcare facilities, community agencies, group and individual settings, community events, and canvasing. This individual will use the Community Health Worker model, as well as other evidence-based crisis and trauma support methods. Provides education, training, and support to children and families as well as makes referrals for other needed services and supports. Work regular schedule with some night and weekend hours.
High school diploma or equivalency and Michigan CHW certification (or complete CHW certification within 1 year of hire) and a minimum of one (1) year of experience working with children and families in a community agency, school, and/or childcare setting providing outreach and supportive services. Employee must maintain Community Health Worker certification to retain employment.
Individual with a strong knowledge of genesee county community resources. A current or previous recipient of mental health services or community resources; or was caregiver to someone participating in these services. Currently holds commercial driver’s license (Cdl) or experience driving recreational vehicles. Has demonstrated experience managing a small caseload effectively (Linking to community resources, ensuring that the individual/Family has successfully connected to that resource).
