Provides crisis intervention, outreach, and case management to adults experiencing behavioral health crises in home and community settings. Collaborates with psychiatric teams to implement treatment plans and connect clients with essential community resources.
Requires an Associate's degree (Bachelor's preferred) and at least 2 years of experience in a mental health or school setting. Must possess a valid driver's license, automobile insurance, and knowledge of behavioral health diagnoses.
Under the supervision of the Emergency Services (ES) Program Manager, provides crisis intervention, outreach services in the home, office and community settings, and case management to adults experiencing behavioral health crises. Collaborates with psychiatric and allied mental health team in assessment, diagnosis, intervention and treatment planning. Assists in conducting risk screenings for adult clients in crisis in clinics, in homes, and in the community. Provides individual interventions to keep the client stabilized as identified in the ES Mobile Team, in the home, at a Center facility, and in the community. Provides case management interventions such as resource linkage, skills building, support, and interventions to clients following a behavioral health crisis to support stabilization. Assists clients in accessing eligible benefits or services and connecting to a natural support network to reduce risk of additional contacts with emergency services. Functions as an advocate and liaison for client with team, community agencies, schools, employment sites and other providers to coordinate care immediately following contact with ES and until regular services are put in place Coaches clients in symptom management strategies. Assists clients in managing prescribed medications, supporting them in attending appointments. Follows up on referrals for services, assisting in transmission of information and records, and in making sure that client gets to scheduled appointments. Maintains frequent communication and collaboration with client and community providers until there is a warm hand-off with a responsible treatment provider. Maintains knowledge of available providers and resources in the community to address specialized needs. Participates in supervision, team conferencing, department and agency meetings. Maintains detailed, accurate, and timely documentation in medical record. Collects, records, and analyzes required data within scope of job responsibilities. Enhances and maintains professional expertise through educational activities and trainings. Engages in trainings on evidence-based practice models specific to their assigned population, including motivational interviewing and cognitive behavioral therapy, implementing learned skills in daily interventions. Meets requirements for client contact time according to treatment plan, and for agency productivity targets. Follows agency policies and standards for client rights and confidentiality. Other duties as assigned MINIMUM QUALIFICATIONS: Associate’s degree, required. Bachelor’s degree, preferred. At least 2 years experience working with in a mental health or school setting NECESSARY SPECIAL REQUIREMENTS: Knowledge of human development and family systems issues. Understanding of mental health and substance use diagnoses Knowledge of community resources for clients and families with behavioral health disorders. Advocacy and coaching skills. Valid driver's license and use of in order to perform the major functions of the job. Automobile liability insurance coverage for vehicle being used in accordance with Center requirements. Ability to work collaboratively as a team to coordinate care for client with intensive behavioral health needs.
