Salary
$34 - $36 / HOUR
Develop and manage individualized care plans while acting as the primary advocate for patients and their families. Coordinate access to medical providers and community resources to improve health outcomes and manage chronic conditions.
Requires a current RN or LPN license in Illinois with at least 3 years of nursing experience. Candidates must have 2+ years of experience in case management and geriatric care, specifically within post-acute or long-term care settings.
