The APP will create personalized treatment plans and manage patient care for chronic disease reversal in a collaborative environment. Responsibilities include overseeing patient progress, interpreting lab tests, and coordinating care with health coaches and other providers.
Requires a Master's degree and active licensure/certification as a Nurse Practitioner or Physician Assistant. Experience in medical dermatology and proficiency with EHR systems are highly preferred.

We are looking for an Advanced Practice Provider (APP) with medical dermatology experience to play an important role in our onsite care team at AndHealth and with our community partners. The APP helps with all aspects of patient care, including diagnosis, treatments, and consultations. They understand that there is no single solution when it comes to patient care and thrive in a collaborative care team environment focused on addressing each patient with a unique, whole-person approach.
