Provide therapeutic emotional support, education, and resource information to hospice patients and their families. Advocate for patient needs and execute the Plan of Care in collaboration with the healthcare team.
Requires a Bachelor's of Social Work with at least one year of experience and a valid Texas State license. A Master's degree and hospice or healthcare experience are preferred.
