The RN Case Manager coordinates healthcare specialists and accompanies residents to medical appointments. They are responsible for managing admissions, readmissions, and ensuring care plans are accurately communicated and documented.
Candidates must hold a current Missouri Registered Nurse license and CPR certification. A minimum of one year of experience in an adult medical/surgical unit is required, with home care experience preferred.

Assist the men with scheduling and coordination of health care specialists Accompany Jesuits to physician or specialist appointments as needed Report changes in condition and relevant health updates to the nurse on duty, Care Coordinator Nurse, and Jesuit leadership Manage admissions and readmission, including communication of condition, treatments, and medications Ensure care plans are accurately and promptly communicated to the Care Coordinator Nurse and/or floor nurse on duty Assume full responsibility for assigned cases Complete documentation, charting, and planning in accordance with prescribed procedures Delmar Gardens is a family-owned, St. Louis-based company that has been serving the community for 60 years.
