Develop and implement housing plans for Managed Care Plan members to help them acquire permanent housing. Coordinate with housing agencies and provide housing-focused case management and community referrals.
Requires a high school diploma with some college or counseling experience and a valid California driver's license. Experience working with homeless populations and proficiency in Microsoft Office are preferred.

The Housing Navigator is responsible for the development of a housing plan in collaboration with Managed Care Plan (MCP) members that will assist them with acquiring permanent housing based on their conditions and needs, providing housing-focused case management, and effectively utilizing community resources via referrals, direct interaction with landlords, and housing management on behalf of MCP members.
