Job description
Moorings Park is looking for a Licensed Practical Nurse for our Home Health team. The Licensed Practical Nurse is responsible for following established policies and procedures and/or specific instructions from the Clinical Manager or Director of Clinical Services. The LPN staff nurse is responsible for providing quality and safe delivery of home health skilled nursing services in collaboration with the RN Case Manager. The LPN provides services under the supervision of a qualified registered nurse.
Contributions
The Licensed Practical Nurse (LPN) provides patient services as assigned and is directly supervised by the Registered Nurse.
Provides specific skilled procedures as assigned and in accordance with acceptable standards for the LPN, including sterile dressing changes, administration of oral, IM, and rectal medication, routine ostomy care, catheter care, observation of signs/symptoms, tube feedings, teaching patient/family in self-care techniques.
Reports patient adverse signs and symptoms, progress toward achieving patient goals, and any questions regarding care to be provided to the supervising RN and/or Clinical Manager.
Prepare current and accurate documentation of treatments, teaching and training, observations, patient responses and reports to RN.
Obtains interim physician’s orders and reports on progress and changes in client condition to RN case manager, MD, Clinical Manager and/or Director of Clinical Services.
Assures that accurate transcription and implementation of physician's orders have been recorded.
Monitors documentation of discharge planning activities along with the Case Manager.
Maintains cooperative manner towards patient/family and all members of the health care team.
Communicates and works cooperatively with patient/family and all members of the health care team.
Attends and participates in In-service programs.
Maintains client confidentiality and works in compliance with HIPAA regulations and standards.
Works in unison with others professionally and respectfully carrying out the Mooring Park Mission and Spirit Statements.

During emergencies or natural disasters may be required to be on duty for the duration of the disaster.
Practical Nurse (LPN) / Vocational Nurse (LVN) graduate from an accredited program required
Active Florida Licensed Practical Nurse/ Licensed Vocational Nurse licensure required
Active Florida CPR Certification is required
Previous geriatric and home health experience preferred
Actively promotes patient confidentiality and rights and works in compliance with HIPAA regulations and standards
A current Florida driver’s license, clean driving record and proof of insurance are required
