Provide direct patient care, including physical assessments and pain management, within home health and hospice settings. Collaborate with healthcare teams to deliver safe, quality care and perform behavioral health evaluations for specific patient needs.
Requires a current California Registered Nurse license, a valid driver's license with an insured vehicle, and BLS certification. Preferred experience includes one year of home health or two years of nursing with at least one year in public health, home care, or hospice.
Provides care in both the home health and hospice settings. Assignments will be throughout Orange County, patient care territories and assignments may change daily.
Utilizes medical/ psychosocial model to individualize care plans based on Physician orders for patients with depression, anxiety, or dementia, with the goal of keeping patients in their homes and out of the hospital. Performs comprehensive behavioral health evaluations of mental status, mood, and behavior.
