Facilitate resident transitions and develop individualized care plans and social histories. Act as a resident advocate while coordinating interdisciplinary service meetings and ensuring compliance with CBRF regulations.
Requires a Bachelor's degree in Social Work or a related field and a current CSW license in Wisconsin. Candidates must have at least one year of experience in a CBRF or long-term care setting.
Bachelor’s degree in Social Work or a related human services field (accredited program). Current Social Worker License (CSW) in the State of Wisconsin. No experience necessary; we are willing to support and train. CBRF Certification (or ability to obtain within 6 months of hire). Strong communication skills (written and verbal) with professionalism and discretion. Proven ability to work independently and manage multiple priorities. Excellent interpersonal, problem-solving, and conflict resolution skills. Knowledge of community resources and how to access them. Basic computer proficiency.
