The Social Worker is responsible for conducting psychosocial evaluations, providing ongoing counsel, and managing bereavement services. They provide physical, emotional, and spiritual support to hospice patients and their families in accordance with a plan of care.
Candidates must hold an MS degree in social work from an accredited school and possess a current state social worker license. At least 2 years of clinical experience, preferably in healthcare or hospice, is required along with reliable transportation.
