Provide skilled nursing, intervention, and supportive care to hospice patients in home and facility settings during on-call shifts. Coordinate patient and family care, perform assessments, and maintain detailed electronic documentation of interventions.
Responsible for providing skilled nursing, intervention and supportive care to patients in the home setting for evening, night, weekend and holiday on-call duty in accordance with the plan of care.
Paid at a salaried rate. Attendance at scheduled staff meetings, orientation or training sessions, or other assigned duties will be paid at the agreed upon hourly rate. Mileage accrued through travel required for the job will be reimbursed at the standard agency rate.
Work Hours and Frequency
The on-call nurse will be available by phone to meet the needs of the patients, as assigned for two 24 hour periods beginning at 8:00 am Saturdays and Sundays. He /She is responsible for identifying and coordinating patient/family care to support patients with advanced illness and families in home, skilled nursing facility or residential care. Facility.
Communication related to on-call interventions will be documented and communicated to the DON with his/her assessment, plan of care changes and on-going need.
Graduate of an accredited school of Nursing. Valid license as a Registered Nurse in the state providing nursing care. Hospice or Home Health experience is preferred. Minimum one year recent professional nursing experience Valid Driver’s License Computer skills
