About the role

Provides palliative and supportive care to patients under RN supervision, including assessing patient needs and formulating care plans. Responsibilities include administering medications, maintaining detailed records, and collaborating with an interdisciplinary team.

Job summary

Provides palliative and supportive care to patients under the supervision of the Registered Nurse and according to physician’s orders.

Qualifications

1. Graduate of an accredited school of nursing. Licensed Practical Nurse.

2. Must be currently licensed (in good standing) in the state where the Hospice Program is located.

3. A least one-year of work experience as an LPN, preferably in an acute care setting.

4. Documentation of good physical and emotional health. Able to lift, push, and pull to moderate loads.

5. Speaks, writes, reads, and comprehends English.

Responsibilities: (including but not limited to):

1. Complies with accepted ethical conduct and professional Standards of Nursing Practice as set forth by the American Nurses Association.

2. Assumes responsibility for a patient/family caseload

3. Assesses patient’s condition and family needs through the collection of health data and the recognition of psychological, social, and spiritual factors that may influence the patient/family plan of care. Re-evaluates patient/family needs as required.

4. Formulates, modifies and carries out a patient/family care plan which prescribes nursing actions toward achievable goals according to patient needs.

5. Affirms a person’s right to choice and control in decision-making. Presents and discusses options with the patient and family and assists them in making informed decisions.

6. Administers prescribed medications and treatments as delegated by the Registered Nurse. Shall not access any intravenous appliance for any reason

7. Initiates and participates in the development of the Plan of Treatment, obtains Physician’s orders, clarifies those orders, and obtains necessary revisions as needed Informs the Physician of changes in patient’s condition and provides written summary reports to the Physician.

8. Maintains a safe, therapeutic environment for the patient.

9. Participates in health counseling and teaching with patient, primary caregivers, volunteers, and employed caregivers.

10. Maintains detailed records of nursing actions, patient progress, and response to therapy and teaching so that care plans can be documented, evaluated, and updated.

11. Attends IDG meetings and facilitates full team discussion, consultation, and evaluation of issues concerning the patient caseload.

12. Documents all patient-related activities in a timely manner.

13. Accepts accountability for own practice.

14. Complies with policies and standard procedures.

15. Communicates information effectively to appropriate personnel to facilitate continuity of care.

16. Maintains competency by participating in in-service and continuing education programs.

17. Reacts to change productively and performs other job-related tasks and duties as assigned.

Physical demands

Able to lift, push, and pull light to moderate loads. Moderately heavy physical effort. May occasionally do heavy lifting, pushing, or pulling.

Requirements

Must be a graduate of an accredited nursing school and a Licensed Practical Nurse in good standing. Requires at least one year of LPN experience, preferably in an acute care setting, and the physical ability to lift moderate loads.
