The In Reach Coach provides education and diversion activities for individuals with serious mental illness to facilitate transitions into permanent supportive housing. They collaborate with clinical teams to ensure whole-person care and link members to essential community resources.
Candidates must have a High School Diploma, a North Carolina Peer Support Specialist Certification, and one year of lived experience with behavioral health or substance use. A valid driver's license and residency within the specified Southern Region counties are required.

Trillium Health Resources has a career opening for an In Reach Coach to join our team! The In Reach Coach is responsible for Diversion and In Reach activities for the TCL Special Healthcare Population with Serious and Persistent Mental Illness (SPMI) residing in Adult Care Homes, State Hospitals, Correctional Institutions and various community settings by providing education about community resources, services, supports and TCL permanent supportive housing. The In Reach Coach ensures members receive whole person care focused on all members’ needs including physical health, behavioral health, and social determinants of health/unmet health resource needs that helps to establish positive outcomes for members and facilitate community integration.
