Provide comprehensive care to patients with endocrine disorders across multiple skilled nursing and long-term care facilities. Coordinate with care providers and analyze patient labs and studies under the supervision of a Collaborative Physician.
Requires a Master's degree in Nursing, a valid NP, PA, or Physician license, and DEA certification. Candidates must have at least 6 months of experience in long-term care and one year of experience in Endocrinology.
Assist care coordinators across the different providers and settings Promote better health outcomes through communication between facilities and patients Assess patients with Endo conditions and report through our proprietary software system You will be cross-trained in other areas of care management Work under the supervision of an off-site Collaborative Physician Gathering and analyzing patient information Detailed review of the system Review of all pertinent patient labs, studies, and provider notes
Minimum 6 months of experience in long-term care or a skilled nursing facility Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license DEA License CPR BCLS/BLS certified Must hold a Master's Degree in Nursing Completed a degree from an accredited Nurse Practitioner program A year of experience in Endocrinology Preferred experience in Documenting EMR (Sigma, Visual, PCC, etc.): training provided Must have great organizational and time management skills. Reliable transportation Must be willing to travel to different nursing homes within the state of Florida (100% commitment)
