Acts as the primary liaison between the hospice organization and hospital systems to coordinate patient admissions and care. Responsible for conducting psychosocial assessments and collaborating with interdisciplinary teams to ensure adherence to the hospice Plan of Care.
Requires a Master's degree in Social Work from a CSWE accredited institution and valid clinical licensure (LCSW/LCSWA/LISW-CP) in North Carolina and/or South Carolina. Experience in medical social work or hospice care is preferred.
